Many agencies face the same reality: their resources are stretched. This is especially true for those mandated to provide services to rural communities like Fort Erie or those not physically located in this area but in other communities like Niagara Falls and St. Catharines.

These realities raise the question, "How can we work together to solve it?" Given our commitment to individuals and families living with brain injuries, diseases, and disorders, our answer was to explore ways to share resources and programming synergies.

In 2016-2017, with funding from the Trillium Foundation, BIAFE led an Advisory Group that discussed this challenge and how to address it. Representatives from Alzheimer Society Niagara, Region of Niagara Senior Citizens, Niagara Health Stroke Recovery Unit, Ontario Brain Injury Association, and the Town of Fort Erie developed a service model.

Our service model offers agencies who want to provide services to Fort Erie residents that align with the below criteria:​

Physical space in BIAFE's building at 649 Niagara Boulevard. This can simply be a desk for a few hours per week or an office or space to hold programming intermittently;

Referral arrangements and/or shared service delivery or case management.
Agencies can access some or all of these options.
This model's intent is to ensure that agencies can connect with Fort Erie residents to provide support and care. Given the ever-increasing number of seniors and persons living with dementia and stroke, the need for agencies to work together has never been more important.

BIAFE is also committed to supporting the Region's long-term care hub being constructed in Fort Erie. By agencies participating in the model in whatever manner best suits them and their consumers, we can offer "The Hub" seamless access to community services.
